This notice explains what information Clinic Hub collects when you request an appointment online, why it is collected, who can see it, and how to reach us with questions. Clinic Hub is the scheduling system used by your community's health program for Cat Lake TytoCare Clinics.
Who is responsible for your information
Your community's health program is responsible for your personal health information and decides how it is used for your care. Clinic Hub is provided to the health program by 1001671897 Ontario Inc., which runs the system on the health program's behalf and may only use your information to operate, secure, and support it.
What we collect
- About you: your full name and date of birth, and the name of the person arranging the appointment if it is someone else.
- How to reach you: the contact method you choose, such as a phone number, email address, or messaging app name.
- Your request: the clinic, date, time, and visit type you choose, standby availability, whether you need transportation, whether the request is urgent, and any notes you add.
- Technical information: the pages you visit, your browser and device type, and a coded version of your internet address. We use this to keep the site secure and to count visits.
Please do not type detailed medical information into the notes box.
Why we collect it
We use your information only to review and confirm your appointment request, contact you about it, arrange transportation if you need it, prepare for your visit, and keep the clinic's records accurate. Staff may contact you using the information you provide to confirm your appointment or follow up about scheduling. We do not sell your information.
Who can see it
- Authorized users.
- 1001671897 Ontario Inc., only when needed to maintain or fix the system, under a written confidentiality and privacy agreement.
Where it is stored and how it is protected
Your information is stored in Canada and protected in line with Ontario's health privacy law (PHIPA).
Cookies
This website only uses cookies that are needed to make the site work.
How long we keep it
Your information is kept for as long as your community's health program needs it to provide care and keep its records, and as required by law. When it is no longer needed, it is securely deleted.
Your choices and rights
You can ask to see or correct your information at any time. You also have the right to contact the Information and Privacy Commissioner of Ontario at www.ipc.on.ca.
